Unlike most North American area codes, 849 covers an entire country: the Dominican Republic. It is the second overlay placed on the original area code 809, following a first overlay, 829, that entered service January 31, 2005. As the combined 809/829 pool approached projected exhaustion by 2008, NANPA approved 849 at the request of INDOTEL, the Instituto Dominicano de las Telecomunicaciones, with assignments beginning July 1, 2009. Mandatory ten-digit local dialing took hold across the full three-way overlay on February 15, 2010.

Because 809, 829, and 849 all serve the same national territory, 849 has no adjacent area codes of its own -- the numbering plan area is the country's borders.
