It was the first overlay ever implemented in upstate New York, and its introduction meant every local call in the region shifted to ten-digit dialing rather than the traditional seven digits. The underlying 315 code dates back to 1947, one of the original North American area codes, so 680 layers a modern fix onto a decades-old foundation rather than replacing it.

The 315/680 territory is large, stretching across an eighteen-county region from western Wayne County to Massena and south toward Cortland. Beyond Syracuse, the primary city, the plan area reaches Utica, Watertown, Rome, and Oswego.
