Miami-Dade County dials under two overlapping codes, and 786 is the newer of the pair, an all-services overlay activated March 1, 1998 to relieve area code 305, which had been running out of numbers as cell phones, pagers and fax lines proliferated after Broward County split off as 954 in 1995. The Florida Keys portion of Monroe County joined the 786 overlay territory on September 1, 2001.

Demand kept climbing, and a second overlay, area code 645, went live August 4, 2023 across that same 305/786 footprint after approval from the Florida Public Service Commission the year before. All three codes now serve the same cities -- Miami, Hialeah, Miami Beach, Coral Gables, Doral -- without any resident ever having had to change their number for numbering relief alone.
